* Proof of identification (photo ID) * Social Security card * Proof of birth dates for taxpayer, spouse and dependents * Wage and earning statements (Form W-2, W-2G, 1099-R,1099-Misc) from all employers * Interest and dividend statements from banks (Form 1099) * Form 1098 to verify any home mortgage interest claimed * Form 1098-T if claiming tuition paid * Copies of last year’s federal and state returns, if available * Bank account routing and account numbers for direct deposit -- bring a blank check * Proof of total paid for daycare provider. Must provide daycare provider's tax identifying number such as their Social Security Number or business Employer Identification Number (EIN). * Summarized list of other income/expenses for those who are self-employed
