Must have/meet: *Income guidelines * Individuals age 50 and older but open to all ages

Please bring with you: * Photo ID * Social Security Card or Individual Taxpayer Identification (ITI) numbers for all individuals on the return * Prior year tax returns (federal and state) with all attachments and schedules and the tax booklet and forms * All W-2's and employment/unemployment statements, and SSA-1099 if Social Security payments received * All 1099 forms (1099-B, 1099-DIV, 1099-INT, 1099-MISC, 1099-R) and any brokerage company statements showing the date and price of acquisition and sale of capital assets such as stocks, bonds * All forms showing federal or state tax withholding if claiming dependent care. Bring information on the dependent care provider including name, Employer Identification Number (EIN) or Social Security number, address and expenses * Complete written documentation for all items being claimed as itemized deductions including medical, interest, tax, contributions and other expenses * Bank account number (if filing electronically)
